Need Advice: In-Floor Circular Brass Electrical Outlets

We are searching to replace 3 of our original In-Floor Circular Brass Electrical Outlets. We want to stick to the original design. Ours have become disengaged with the floor leaving the wires exposed.

----> Does anyone have any ideas on where to purchase these?

So far we consulted, http://www.smarthomes.com but their product is TOO long for our slab. Their product is indicating 7 1/4 depth while our existing space is only about 4 inches deep.
